Depending on the part of the large intestine involved, it can take different names (on the ascending, transverse, descending, cecostomy, and sigmoidostomy). In the case where the colon is dissected and is abutted to the abdominal wall it is called terminal. On the other hand, when the colon is not completely dissected, but only brought out of the abdominal wall, it is called lateral. It is both temporary and permanent.

Ileostomy
It is performed when the entire colon (large intestine) must be avoided by intervening in the ileum, the final part of the small intestine. It requires special skin precautions because the material that comes out is particularly aggressive to the digestive enzymes it contains. This type of ostomy can also be either temporary or permanent.

Urostomy or Urinary Ostomy
This procedure diverts urine from the normal urinary system to an external pouch through a part of the intestine, in most cases with the small intestine, but sometimes with the large intestine. It is generally permanent.

Each of these procedures has its own specific management procedures, but each of them is manageable on a daily basis on its own with the proper training, attention, and medical support, ensuring a good quality of life.

Daily management of Ostomy

Daily ostomy management requires proper attention to ensure hygiene first and foremost, but comfort is also a key component that should not be overlooked.
Here are some important attentions for the daily routine:

Hygiene and cleanliness

Cleaning the Stoma
The stoma area should be kept clean and dry, using lukewarm water and mild soap for cleaning, avoiding products containing alcohol, oils, or perfumes that could irritate the skin.
Gently pat dry with a soft towel or gauze.

<a hygiene

Frequency of pouch change and maintenance
The frequency of change depends greatly on the type of ostomy and your daily activities, but it is generally recommended to change the pouch every 2-3 days or at least when it is one-third full, so as to avoid excessive weights that could cause detachment or leakage.

Checking for leaks
Check regularly, but without compulsiveness, that the closure system is secure and that there are no leaks around the stoma or the edges of the bag, creating a routine that is most congenial to you.
This will ensure that you maintain peace of mind and avoid irritation or accidental spills.

Bathing and showering with an ostomy<br Sharing experiences and advice can be very helpful in dealing with daily challenges.
We will discuss this in detail later in our guide.

Nutrition and Diet

Nutrition and diet play a not insignificant role in ostomy management, profoundly affecting function.
In addition to being important, it is critical to follow specific nutritional guidelines to ensure that the diet minimizes any complications.

What are the recommended foods

Low-residue foods
Foods that leave less residue, such as white rice, pasta, white bread, and lean meat, may reduce the risk of stoma blockage.

Well-cooked fruits and vegetables
Cooked fruits and vegetables are easier to digest, which is why they should be preferred. Avoid the skin of fruits and vegetables to reduce the risk of occlusion.

Protein
Always be sure to include a good amount of protein in your diet to promote healing and tissue maintenance, choosing sources such as chicken, turkey, fish, eggs and certain types of legumes.

What foods are to be avoided or limited

Gas-producing foods
These foods that increase gas production already cause discomfort in people who have not undergone the procedure; the problem is exacerbated for ostomates.
Therefore, cabbage, broccoli, onions, beans, and carbonated beverages should be avoided.

Foods that can cause occlusions
Foods that are difficult to digest, such as mushrooms, corn, nuts, and seeds, can increase the risk of stoma blockage and the significant complications it entails. Avoid them!

Foods and drinks that promote dehydration
Coffee, tea, and alcohol can increase urine production and lead to dehydration, especially for those with an ileostomy.
These should also be avoided!

Food and drink: please note!

Hydration
Maintain adequate hydration by drinking plenty of water throughout the day, especially if you live in a hot climate or are physically active.
This will facilitate evacuations.

Monitoring
Keep track of your response to foods by introducing one at a time into your diet and noting any effects on your ostomy or general well-being, excluding the most problematic ones and creating a routine for those that can aid in managing evacuations.

Nutritional counseling
Consider consulting a dietitian or nutritionist who specializes in ostomy management so that you can develop an eating plan that meets your nutritional and health needs and is compatible with your routine.

Physical activity and sports

Maintaining an active lifestyle is essential for anyone, including ostomy wearers. However, it is important to choose suitable exercises and take precautions to avoid complications.

Here are some tips for staying active safely:

Before starting the sports activity

Ask your doctor
Before starting any exercise program, it is essential to consult with your doctor or a trained therapist.
They will be able to provide personalized advice based on your specific physical condition and type of ostomy.

Listen to your body
Activity should be started slowly, gradually increasing the intensity so that you pay attention to how your body reacts and stop promptly if you feel pain or discomfort.

Recommended exercises for ostomy wearers

We present here some activities that we know can make the case for those who are stomized.

Walking
Walking is a great starting point for anyone becoming familiar with physical activity, including ostomy patients.
It is a low-impact exercise that can be easily adapted to the times and spaces of available.

Swimming
Non-competitive swimming and water aerobics are excellent activities because the water supports the body, reducing stress on the stoma.
Just be sure to use a stoma cover, waterproof if necessary.

Cycling
Riding a bicycle, either outdoors or on an exercise bike, can be a good choice, as long as you feel comfortable and the pressure on the stoma area is minimal.

Stretching exercises and yoga
These activities can help improve flexibility, reduce stress and strengthen muscles without putting too much pressure on the stoma area.

Exercises at home
At home you can do some simple exercises that can help you keep fit and manage your ostomy.

You can find the dedicated article we made here:
Ostomy: the best 12 exercises to do at home

Tips for avoiding complications

Stoma support
Consider using a band or ostomy support during physical activity to protect the area and keep the pouch in place.

Avoid high-impact sports activities
Contact sports or activities that involve sudden movements or direct pressure on the stoma area should be avoided unless explicitly approved by your physician.

Hydration
As already recommended, especially during exercise, be sure to drink plenty of water before, during and after exercise, especially if you have an ileostomy, as you are more susceptible to dehydration.

Finally, remember that the main goal is to maintain an active and healthy lifestyle. As difficult as it may seem in the early days to overcome difficulties and embarrassments, have confidence in your ability to use advice and experience to be ready for any situation.

Peaceful and open communication

Honest Dialogue
Communication is essential in any relationship, it is even more so for those living with ostomy and the people you are dating.
Being open about your ostomy, especially with a potential partner, helps build understanding and acceptance that is fundamental to any romantic relationship, as well as understanding with the genuineness of the person you are dating

Timing
Timing is important in choosing when to talk about your ostomy. As mentioned earlier, parents can play a proactive role in ensuring that the child does not feel excluded or different. This can include discussion with teachers and classmates and, in general, all educational or socializing figures important in the child’s life, so as to promote an inclusive and inclusive environment.

We particularly recommend building the best possible relationship with the school, where the child spends much of his or her time, in a spirit of cooperation and mutual support, inquiring about all the resources that can be put in place to improve the quality of school life for the child and the class in which he or she will be placed and their respective teachers.

The emotional aspect of caring for a child with an ostomy cannot be the sole preserve of the family, which may also risk feeling alone or burdened beyond its resources, descending into anxious and overprotective states.

Seeking support groups and online resources can provide additional layers of understanding and community.
These spaces offer parents and children the opportunity to share experiences and strategies for dealing with similar challenges, creating a sense of normalcy and belonging.
In addition, constant guidance from professionals is guaranteed to ensure that the child has an effective tool in times of difficulty and to structure his or her person fully, without the ostomy being a ballast.

Finally, preparing for unforeseen events is an essential part of care. Having an emergency ostomy kit, including extra supplies and other essentials, provides peace of mind when away from home.
This allows your child to attend outings, birthday parties, and vacations with fewer worries.

We leave you to our in-depth article with our 12 tips:
12 useful tips for your baby’s ostomy care

Solutions to common problems

Living with or being a caregiver to an ostomy involves managing several daily challenges, many of which can be overcome with the appropriate strategies, as we saw earlier.
We now present some of the common problems that caregivers may encounter, accompanied by practical solutions for dealing with them.

Irritation and skin changes

Problem

The skin around the stoma may become irritated or damaged due to prolonged contact with feces, urine, or bag adhesive.

Solution

Keeping the area clean and dry is essential.
